Sioux Lookout Diabetes The SLDP was established 11 years ago and is a member program of The Northern Diabetes Health Network in Thunder Bay Ontario, Canada. Our team presently consists of 3 Diabetes Nurse Educators, 2 Diabetes Dietitian Educators and an Office Administrator.
