Northern Diabetes Network

The Northern Diabetes Health Network (NDHN)

The Northern Diabetes Health Network (NDHN) is an evolving health care organization committd to enhancing iabetes services for people in Ontario.  This organization, funded by the Ontario Ministry of Health and Long-Term Care, offers a broad range of diabetes speciality prorams to meet the needs of people in communities of northern Ontario, as well as specialized programs focusing on chldren and young people in selected communities across the province.

Canadian Diabetes Association

The Canadian Diabetes Assocation is the national organization heading the fight against diabetes by helping people with diabetes live healthy lives while trying to find a cure.  Their mission is:

  • providing people with diabetes and healthcare professionals with education and services
  • advocating on behalf of people with diabetes
  • supporting research
  • translating research into practical applications
     

Sioux Lookout Diabetes

The SLDP was established 11 years ago and is a member program of The Northern Diabetes Health Network in Thunder Bay Ontario, Canada. Our team presently consists of 3 Diabetes Nurse Educators, 2 Diabetes Dietitian Educators and an Office Administrator. We visit the remote northern communities by small plane monthly, as well as a walk in centre located in downtown Sioux Lookout.

National Aboriginal Diabetes Association

NADA's Mission is to be the driving force in addressing diabetes and Aboriginal people as a priority health issue by working together with people, Aboriginal Communities and organizations in a culturally respectful manner in promoting healthy lifestyles among Aboriginal people today and for future generations.
